Birth registration form collection services by the Johannesburg Department of Home Affairs

Please be advised that Genesis Maternity Clinic holds NO responsibility for any errors on birth certificates issued by the Department of Home Affairs. All errors and changes need to be handled directly between the parent and the Department, Genesis Maternity Clinic staff members are not available to handle these matters on your behalf. We thank you for your understanding in regards to this matter.


You need to register your baby at the department of home affairs and apply for a birth certificate within 30 days of baby’s birth.

Option 1

Visit your nearest home affairs and complete the documentation provided there.  the mom, dad and baby need to be present during the application.

Documents to submit with the application:

  • Copies of your ID’s and marriage certificates (where applicable)
  • Maternity certificate (supplied by Genesis Maternity Clinic)

Option 2

An application form for your baby’s certificate is available for you to complete at Genesis Maternity Clinic. This service is only for those clients who have birthed their baby at Genesis Clinic.

Ensure that the correct supporting documents are submitted with the application form at reception:

  • Copies of your ID’s and marriage certificates (where applicable)
  • Maternity certificate (supplied by Genesis Maternity Clinic)

This service is offered by the department of home affairs. An official from the department of home affairs will collect the completed documents once a week.

Your baby’s birth certificate will be delivered to Genesis Maternity Clinic. Kindly follow up by contacting the clinic at 011 544 9800 to enquire if your baby’s birth certificate is ready for collection.

Please note this service is provided by the department of home affairs and they only do the normal birth certificates for Genesis Maternity Clinic and not the unabridged birth certificates, you will need to visit your nearest Home Affairs for this service.
